LOLIN ADOPTION APPLICATION

This questionnaire is for your benefit as well as ours. The information you provide will enable us to   better determine if a Lab is the right breed for you, and which of our dogs will fit best into your lifestyle. Please be honest in your response as we will be honest in ours. If we do not feel you can provide a proper home for a rescued Lab, we will tell you why.

The Lab you may want may not have the personality you need, and if that is the case, we will tell you why as well.  Allow us to do our job by determine which Lab is for you.  Remember, that adopting a dog is a life long commitment. If you doubt that you can make such a commitment, please stop here.

Do not put one of our Labrador Retrievers through the trauma of rejection from yet another home.  This is why WE match you with the dog YOU need.  WE DO NOT GIVE REFUNDS FOR OUR DOGS! 

Our adoption fee helps to cover the cost of pulling the dog from the shelter, updating their vaccines, spaying/neutering the dogs, heartworm testing, and providing any medical need to prepare them for adoption.

Please REMEMBER, we are a 100% volunteer, Public Charity organization that receives NO pay and we really do this just for the Love of the Labs. We all have our own personal lives, most work full time and raise families as well, and we try to do the best we can, but this process can take time, so please be patient.  You should hear back from us within 48 hours from the time you sent in your application, if you do not hear from us in that time, please email us at  lolin@comcast.net and ask why.

All of the information I have given above is true and complete. Any dog I may adopt will reside in my home as a pet. I will provide it with good clean, fresh food and water, secure shelter, training, affection and medical care.

I understand that Love of Labs, IN or authorized agents are not responsible for the accuracy of information received about the temperament, habits and physical conditions of dogs available for adoption. I understand that it is my responsibility to see and evaluate the dog for myself before agreeing to adopt it from LOLIN, and any person associated with its adoption efforts, is in no way liable or responsible for any damage, accident or injury resulting from the placement of a dog into my household. I also understand that I will be required to sign an adoption contract to place a dog in my home. I am in full agreement with these terms of adoption. By electronically initialing, I acknowledge that I have read and understand this paragraph. I also understand that this adoption can take up to 2 - 4 weeks. Furthermore, I understand that LOLIN is a non-profit, 100% volunteer non-profit public charity. LOLIN does NOT give refunds.
